Event Description
It was reported that the trial head could not be removed from the stem during the provisional reconstruction.Delay about 10 minutes.No consequences or impact to the patient.Due diligence is in progress for this complaint; to date no additional information or product has been received.

(b)(4).Investigation of this incident is currently ongoing.Once the investigation is completed, a supplemental medwatch 3500a will be submitted.

(b)(4).This follow-up report is being submitted to relay additional and/or corrected information.Stem was returned for investigation.The body of the stem is inconspicuous.The neck of the stem shows some scratches.On the taper of the stem, as well as on the contact line between neck and taper, it is possible to see some nicks.These damages were most likely caused in the attempt of removing the trial heads from the stem.Review of the manufacturing records identified no deviations or anomalies during manufacturing.Device used for treatment.Medical records were not provided.With the available information a definitive root cause cannot be established.If any further information is found which would change or alter any conclusions or information, a supplemental report will be filed accordingly.Zimmer biomet will continue to monitor for trends.
