Event Description
It was reported that, during a tka procedure, the real intelligence robotic drill's silver ring was not lined up and would not accept the drill attachment, once it was repositioned, the drill attachment locked on.However, a communication failure: please contact robotics customer support error was received and the drill would not calibrate.Surgery was completed after a non-significant delay, with a back-up device.Patient was not harmed.

H3, h6: the real intelligence robotic drill, part # rob10013, serial # sn000030, used for treatment, was returned for evaluation.The reported problem was visually confirmed.The silver rotating collar has been bent inward at both of the insert wells and the thread seal cap is slightly dented.A functional evaluation was performed.The reported problem was confirmed.It was found that the carriage assembly was damaged and could not retract fully into the drill.The most likely cause seems to have been damage to the thread seal cap and collet assembly of the carriage, possibly due to impact trauma against the ground or another rigid object.A review of manufacturing and service records indicate the device met all specifications upon release into distribution.A complaint history review for similar reported/confirmed complaints found similar events.A historical escalation event review was completed.A review of prior escalation actions found no actions applicable to the scope of the reported complaint.The failure mode and associated risk have been anticipated within the risk file and the documented risk level is still adequate.Although no further containment or corrective action is recommended or required at this time, the failure mode will continue to be closely monitored through complaint investigation and trended through post market surveillance activities.
