The device was evaluated by olympus.The evaluation found that the allegation was not confirmed.Additional issues were identified during the device evaluation: the visibility was cloudy; the flexible tube of the insertion section was crushed; the bending section adhesive part was missing; paint was floating on the operation part; the coating on the gripping part was lifted; and scratches were found in multiple locations on the device.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation.
 
Event Description
The olympus representative reported that half of the display screen on the uretero-reno fiberscope was dark and segmented.No patient injury or procedure impact was reported.During the evaluation of the device, it was noted that the forceps stopper cap was scraped off.This report is to capture the reportable malfunction of the scraped-off forceps stopper cap noted at estimation.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, the root cause of the reported event is unable to be determined.However, the cause of the event is likely due to stress of repeated use, external factors, or handling.Olympus will continue to monitor field performance for this device.
