Event Description

Medtronic received information regarding an imaging system being used during a procedure.It was reported that the system took anterior posterior (ap) scout images before it showed an error that the system could not initialize the collimator.The medtronic representative (rep) was present during the case and tried rebooting the system, but it was stuck in initializing for approximately 15 minutes before the initializing collimator error appeared again.Rebooting the system numerous times did not resolve the issue.No further information was provided.

Medtronic received information regarding an imaging system being used during a sacroiliac and thoracolumbar procedure.It was reported that during a case, the gantry was unable to open.They tried to reboot the system and the issue was not resolved.The site had to manually open the system.They were able to close it with the pendant then got a collimator error.
