Manufacturer Narrative
An evaluation of the returned device confirmed the customer allegation.Connecting tube had a dent.There were few additional findings.Adhesive on bending section cover was chipped.Connecting tube was wrinkled.Due to wear of angle wire, bending angle in up direction does not meet the standard value.Electrical contact of video connector was shaved.Label on light guide connector was peeled.Universal cord was wrinkled and dented.Scratches were found throughout the device.The investigation is ongoing and follow up with the user facility is currently being performed.A supplemental will be submitted on completion of investigation or if any additional information is available.
 
Event Description
The company representative on behalf of the customer reported, the image guide tube of the loaner cysto-nephro videoscope was corrugated.The device was returned, and an evaluation revealed scraping of the forceps plug cap.This report is being submitted to capture the reportable malfunction found during evaluation.There were no reports of patient harm associated with the event.
 
Event Description
It was corrected that the device was owned by the manufacturer; there was no reported complaint from users who had used this device.The suggested phenomenon was confirmed on inspection after it was returned.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 14 years since the subject device was manufactured.Based on the results of the investigation, a definitive root cause could not be established.The suggested phenomenon was presumed to have been due to stress, handling, or other factors.Olympus will continue to monitor field performance for this device.
