Event Description
|
Olympus was informed that while using a resection electrode, in the second half of the case-use procedure, when the doctor was incising the mucous membrane with cut output, a problem occurred in which the blue and yellow at both ends of the loop gradually moved inward and the loop itself became narrower.The device was replaced with a new product and completed the procedure.No death, injury or harm has been reported to olympus.

Corrected fields: b5/d1/d4/d9 (information was inadvertently omitted from the initial), g2 (add other and remove health professional).This report is being supplemented to provide additional information based on the legal manufacturer's final investigation and device evaluation.The device was returned to olympus for inspection, and the customer's complaint was confirmed.The loop electrode at the tip was deformed.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, the root cause could not be determined.It is likely the reported event occurred due to the use of excessive force from improper handling.Olympus will continue to monitor field performance for this device.
