Event Description
According to the reporter, during a laparoscopic sleeve gastrectomy, in the first firing of the staple on the stomach, the stapler fully fired but the distal staples of the staple line were malformed.The knife blade also did not cut through the small distal portion of the reinforcement material.A new reload was placed on the handle, and was fired over the area where the staples did not form. no leaks shown during leak test of staple line at the end of the case.
