Event Description
It was reported that there were air bubbles detected in two (2) em2400 valve sets.During setup, air bubbles were observed in the valve set.Tech support assisted the customer with replacing the set with a new one of the same lot; however, air bubbles were detected again.To resolve this issue, the set was replaced with a new one from a different lot and the customer was able to continue with setup.There was no patient involvement.No additional information is available.

Additional information was added to h4, h6 and h10.Correction to b5 and f10/h6: medical device problem codes (update a1415 to a16).B5: upon further review, the valve set triggered bubble detected alarms on the automated compounding device; there was no mention of bubbles in the set.H4: device manufacture date: august 31, 2022 - september 2, 2022.H10: a batch review was conducted and there were no deviations found related to this reported condition during the manufacture of this lot.The devices were not received for evaluation; therefore, a device analysis could not be completed.However, this issue did not result in a leak, contamination or accuracy issue and is not likely to cause or contribute to a death or serious injury.Should additional relevant information become available, a supplemental report will be submitted.
