Event Description
It was reported that during an cholecystectomy procedure, when the surgeon went to close pouch and pull it out, the string came off the pouch.The bag stayed inside the patient.The surgeon had to make a larger incision in order to retrieve the bag.There were no adverse consequences for the patient.

(b)(4).Batch # x96a24.Additional information was requested and the following was obtained: all components were retrieved from patient.Laparotomy was performed to retrieve bag from abdominal cavity.Device has already been mailed back.I believe bag was disposed.No changes to post op care.Thank you".Investigation summary the product was returned to ethicon for evaluation.Visual inspection and functional testing were conducted on the returned device.Visual analysis of the returned sample revealed that the pouch device was returned fully deployed.The bag was not returned for analysis.Upon evaluation, the suture was torn.In order to evaluate the internal components the device was disassembled.Upon disassembling of the device no anomalies were noted with the internal components.No conclusion could be reached as to how this damage occurred.As part of ethicon¿s quality process, all devices are manufactured, inspected, and released to approved specifications.A manufacturing record evaluation was performed for the finished device batch and lot number, and no non-conformances were identified.
