The product investigation was completed.Device evaluation details: visual analysis revealed a reddish material inside the pebax surface, and exposed internal components were observed.A microscopic examination was performed in the pebax area.It was observed broken part on the pebax surface.For this observed issue, sem analysis was performed to confirm the damage observed in the affected surface, and it was confirmed that damage was caused from outside to inside; however, the damage is unknown.The issue reported by the customer was confirmed.Since was observed reddish material inside the pebax area and this was partially broken.All units are inspected before leaving the facility as there are functional tests and inspections at control points based on the process flow diagram (pfd) per its part number to avoid this type of damage from leaving the facility.A manufacturing record evaluation was performed for the finished device 30893021l number, and no internal action was found during the review.If additional information is received regarding this event, a supplemental 3500a report will be submitted to the fda.Manufacturer's reference number: (b)(4).
 
Event Description
A patient underwent an ablation procedure with a thermocool® smart touch® sf bi-directional navigation catheter for which biosense webster¿s product analysis lab identified reddish material inside the pebax surface, and exposed internal components.The finding was identified on (b)(6) 2022.It was initially reported by the customer that blood was found inside the tip of the stsf catheter.The physician did not feel comfortable continuing with this catheter.To troubleshoot the catheter was replaced and the issue was resolved and the procedure continued.No patient consequences were reported.The foreign material inside of pebax (blood) is not mdr-reportable.Hole in the pebax is mdr-reportable.
