Event Description
The customer reported to olympus, the evis exera iii xenon light source had a malfunction where the lamp kept getting hot.The issue was found during an unknown procedure.There were no reports of patient harm.
 
Manufacturer Narrative
The device was returned to olympus for evaluation and the customer's allegation was confirmed.The device evaluation found the socket slider switch was worn out, the non-olympus lamp had reduced illumination, the air pump tubing had water invasion, the lamp was overheating due to heavy dust inside the unit and the lamp housing fan, and the lamp had insufficient thermal grease.The investigation is ongoing and follow up with the user facility is currently being performed.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, the definitive root cause of the water invasion inside of the air pump tubing could not be determined.Additional information about the event was requested, but not received.Olympus will continue to monitor field performance for this device.
