Event Description

Venaseal closure systems were being used to treat the great saphenous vein (gsv) and the short saphenous vein (ssv).The lumen was flushed prior to use.The ifu was followed.A guidewire was used for insertion of the catheter.It was reported from sfj to the knee region the segments were properly closed by venaseal, but then the blue introducer had gotten stuck at below the knee region.The physician had tried to remove it, but it got broken, and it was left inside the gsv.Post-procedure, some part of the blue introducer is still intact in the gsv. as it was a bilateral case, 2 kits of venaseal were used.This complication happened when the 2nd leg was being treated.The procedure was completed by leaving behind the blue introducer inside the vein.The blue introducer has been surgically removed after a couple of days of venaseal procedure.No further patient injury report for this event.
