Event Description
The uretero-reno videoscope was returned as part of the asset return process.During routine inspection of the device by olympus, the forceps channel port was found to be shaved.There was no procedural or patient involvement associated with this event.This mdr is being submitted to capture the reportable malfunction found during the device evaluation.

The device was returned and evaluated as part of the asset return process.In addition to the forceps channel port found to be shaved, there was also a foggy image due to damage of the charge-coupled device.The investigation is ongoing, and a supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.

This report is being supplemented to provide additional information based on the legal manufacturer's investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, the root cause of the shaved port was the accessories or metal part of cleaning brush touched the biopsy channel port when the user inserted/withdrew those products.Dirt remained on the subject device due to incompletion of reprocessing, because the control section leaked.Olympus will continue to monitor field performance for this device.
