Event Description

Medtronic received information regarding an imaging system being used during a spinal procedure.It was reported that the imaging system states that the door was open.It was noted that the site could open and close the door but the pendant still read that the door was open.Due to this, the site was unable to take images.There was no impact on patient outcome and the issue caused a less than 1 hour delay.The use of the imaging system was aborted. 2023-feb-07 interface update (rep): additional information was received, it was reported that the cause of the issue was able to be identified over the phone.When the gantry was in an open position and not around the patient, the representative on site taught the site on how to manually open the door.A wrench was used to rotate the source and detector.It was noted that in an attempt to close the gantry while the detector was not in its home position, the doors would not close all the way.It was instructed that the detector was moved back to its home position and the door closed properly.During this troubleshooting, it was noticed that a cover was damaged.
