The subject device was received and evaluated.Device evaluation found the device connecting tube is perforated and metal can be seen however, metal protrusion was not observed.Inspection noted there were no metal parts sticking out from the bending rubber.Furthermore, the following defects were identified during device inspection: connecting tube deformed and perforated.Light guide lens cracked.Grip unit discolored.F.Knob (forceps knob) broken.Ks (lever) mouthpiece deformed.Based on device evaluation findings, inspection found that connecting tube is perforated and metal can be seen however, there is no metal protrusion.Multiple follow ups were made to gather additional information regarding the event reported, however, were unsuccessful , no response is received form customer.The replacement of the distal end, the connecting tube unit, the fe-lever, the ks-mouth piece and the k-mount unit are required as a major repair to return the instrument to the original equipment manufacturer (oem) specification.Investigation is ongoing.This report will be supplemented accordingly following investigation.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.Three attempts were performed to obtain additional information, but no response was received from the customer.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, the root cause of the reported event is unable to be determined.Olympus will continue to monitor field performance for this device.
