Event Description
The olympus representative reported (on behalf of the customer) that during reprocessing the visera elite xenon light source while working with the visera elite video system center, could not be inserted into the scope socket.The customer suspected that the light guide connector on the scope may be broken inside.The intended diagnostic procedure was completed successfully with the same device with no reported delay or patient impact.
 
Manufacturer Narrative
The device was evaluated by olympus.The evaluation found that the allegation was confirmed due to an output connector that was broken and could not be connected.The socket was found to have scratches and foreign material.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.Based on the results of the investigation, it was determined that the phenomenon, ¿scope cannot be inserted¿, was replicated and was caused by the presence of scratches and foreign substances in s socket s40pu (gn231700).No other information could be obtained.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Olympus will continue to monitor field performance for this device.
