Event Description
|
The customer reported to olympus, during reprocessing and ejection, the endoscope reprocessor exhibited error e01 indicating filling the basin with water takes too much time and error e73 was noted during compounding indicating water supply was abnormal.It was noted the cause of the reported issue was weak water pressure.There was no harm or user injury reported due to the event.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, and the device not being returned for evaluation, the definitive root cause of errors e01 and e73 could not be determined.It is possible that the event occurred due to the user not reading the instructions manual carefully and a user mistake in managing the water filter's replacement.The instruction manual identifies the following verbiage, which may have prevented the phenomenon: ¿replace the water filter periodically, at least once in one month to prevent contamination of the rinse water.¿ olympus will continue to monitor field performance for this device.
