Event Description
The customer reported to olympus that the evis exera iii xenon light source was displaying error code e315 (incompatible scope error) when connected to the scope.The scope detection did not work.There were no reports of patient harm associated with this event.
 
Manufacturer Narrative
An olympus field service engineer (fse) was dispatched to evaluate and repair the device.During the visit, the fse spent time investigating and troubleshooting the reported issue.While checking the socket on the light source, the fse noticed a considerable amount of dust and dried residue on the contact points.The fse removed the excess dust and cleaned the contacts in the socket, which corrected the issue with the light source.The subject device referenced in this report was not returned for evaluation, therefore the device evaluation could not be completed at this time.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 9 years since the subject device was manufactured.Based on the results of the legal manufacturer's investigation, definitive root cause of the dust and dried residue on the connector contacts could not be identified.Olympus will continue to monitor field performance for this device.
