Event Description
The customer reported that while performing an ureteroscopy procedure, the angulation on his olympus uretero-reno videoscope became locked and could not disengage.According to the initial reporter, he changed to another unit to complete the procedure without any patient harm.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, the root cause of the reported scope angle lock issue could not be determined, as the device was not returned for evaluation, however, it is possible that there was a problem with the angle wire sliding in the device.The event may be detected by following the instructions for use section below: ¿instruction manual olympus urf type v chapter 3 preparation and inspection/3.2 inspection of the endoscope/inspection of the bending mechanism¿.Olympus will continue to monitor field performance for this device.
