Event Description
The customer reported to olympus, water leaked from the output connector socket from the evis exera iii xenon light source.The issue was found during preparation for use.There were no reports of patient harm.
 
Manufacturer Narrative
The device was returned to olympus for evaluation and the customer's allegation was confirmed.The device evaluation found peeling, cracked, and damaged front label.Liquid invasion.After inspecting customer unit test scopes and video processor (gif-h180, gif-h190, cf-hq190l, gif-q180, ltf-s190-5, cv-190) found error code 200 on screen.This was due to turret function which was damaged.Turret first wheel hitting lens base.Also found worn out scope socket slider switch which caused intermittent use of high intensity mode.Damage inside scope socket.Lens base was broken.Bottom and front panel chassis was deformed.Lamp door and top cover were a little rusty.Lastly, non-olympus lamp was used with meter reading at 247+hours, light output measured out of range.Using old version main power switch which needed to be upgraded.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation.

This report is being supplemented to provide additional information based on the legal manufacturer's investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 9 years since the subject device was manufactured.Based on the results of the legal manufacturer's investigation, a definitive root cause of the peeled/cracked/damaged front label leading to liquid invasion could not be identified.Olympus will continue to monitor field performance for this device.
