Event Description
It was reported the that the slap hammer threads (cross threaded) and osteotome head (bent, chipped) were identified as damaged by cssd staff.No surgeon affiliated with event.All three instruments are part of a consignment set.Surgery was not delayed.

Product complaint # (b)(4).Investigation summary: the device associated with this report was returned to depuy synthes for evaluation.Visual examination of the device revealed that the distal tip was heavily deformed.However, no breakage condition was observed, therefore the reported allegation can be partially confirmed.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.Device history lot: the product investigation found no evidence suspecting an error in the manufacturing or material that would be a contributing factor in the reported allegation(s).A manufacturing records evaluation (mre) was not performed.
