Event Description
The customer reported to olympus, the automatic light control did not work properly on the evis exera iii xenon light source.The issue was found during a procedure.The patient's examination was not affected by the malfunction.Inspection and testing of the returned device found the output connector was broken and could not be connected.The output socket was cracked.There were no reports of patient harm.This medical device report (mdr) is being submitted to capture the reportable malfunction found during evaluation.
 
Manufacturer Narrative
The device was returned to olympus for evaluation and the customer's allegation was not confirmed.The device evaluation found deposits on the lamp.The lamp used was a third party lamp.The scope connector could not be connected securely due to cracked output socket.Additional information has been requested regarding this event.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation.

This report is being supplemented to provide additional information based on the legal manufacturer's investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 4 years since the subject device was manufactured.Based on the results of the legal manufacturer's investigation, a definitive root cause of the cracked/broken output socket could not be identified.However, it¿s likely the cause is related to excessive stress being applied to the output socket.Olympus will continue to monitor field performance for this device.
