Event Description
|
This is filed to report inability to straighten it was reported this was a mitraclip procedure to treat degenerative mitral regurgitation (mr) with a grade of 4.It was noted the patient a severely tortuous iliac vein leading into the inferior vena ceva (ivc).The steerable guide catheter (sgc) was inserted, but when applying +/- knob, the sgc was unable to curve or straighten.Therefore, the sgc was removed.Upon removal, it was observed the shaft was deformed and the tip was kinked.No additional devices were attempted and the procedure was discontinued.There were no adverse patient effects and no clinically significant delay in the procedure.No additional information was provided.

The device was not returned for analysis.A review of the lot history record revealed no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no similar complaints reported from this lot.The investigation determined the reported unable to curve, unable to straighten, difficult to advance, and deformation due to compressive stress of the sgc appear to be related to patient morphology/pathology.There is no indication of a product issue with respect to manufacture, design or labeling.
