Event Description
Medtronic received information that prior to use of a mc2 two stage venous cannula, it was reported that while delivering an order, a defect in the package was noted and while inspecting further, it was noted that the cannula was not straight like the usual ones.The sales rep checked the rest of the carton and found another cannula with the same defect.The devices were replaced.There was no patient involvement, so no adverse effect occurred.
