Event Description
It was reported that the bd posiflush¿ pre-filled saline syringe experienced foreign matter in fluid path.The following information was provided by the initial reporter: the material department reported to the dealer, saying that when the product was inspected before use, it was found that there was a black foreign matter near the salt water in the pre-fill and screw interface; samples can be returned with photos provided;.

H6: investigation summary it was reported there was a black foreign matter near the saline in the pre-filled syringe and screw interface.As a sample was not returned, a thorough sample investigation could not be completed.To aid in the investigation, one photo was provided for evaluation by our quality team.The photo shows a syringe with no packaging flow wrap.The syringe has discoloration at the bottom of the syringe barrel.No other defects or imperfections were observed.A device history record review was completed for provided material number 306595, lot 2096966.The review did not reveal any detected quality issues during the production of this lot that could have contributed to the reported defect.There were no related quality notifications.All processes and final inspections complied with specification requirements.To date, there have been no other similar events reported for this lot.Based on the investigation and with the photo sample analysis the symptom reported by the customer is confirmed, but without the physical sample analysis a probable root cause could not be offered.
