Event Description

Information was received from healthcare provider (hcp) via a manufacturer representative regarding a patient having tlif for unknown indication.It was reported that the driver broke. there were no fragments of the implants or instruments remained in the patient's body. dura repair was done as the broken driver caused a breach. product will be returned.There were no patient symptoms or complications reported as a result of this event.Additional information obtained by translation.Physician was doing a tlif and also had the reduction screws (rmas).When he wanted to put the reduction set screws, the set screw handle did not hold the set screws well and the handle slipped and caused a dura mater breach.This prolonged the surgery as physician had to repair the breach.So far the patient does not seem to have any neurological sequelae.
