After the fse confirmed the customer's allegation, the device was returned to olympus for evaluation.The repair center evaluated the device and confirmed the allegation.The no image was caused by a faulty power supply.It was noted that the issue occurred due to a defective power supply.It was also noted that circuit board was not detecting and the locking mechanism was not working properly.There was corrosion found on the flat cable and the receptacle was found loose.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Event Description
The customer reported to an olympus field service engineer (fse) that the evis exera ii video system center had no image from the monitor.The fse confirmed the issue and also reported finding that the light-emitting diodes (led's) were all glowing on the front panel.The issue was found during preparation for use for a diagnostic endoscopy and it was completed with a similar device.There were no reports of patient harm or procedural delay associated with the event.

This report is being supplemented to provide additional information based on the legal manufacturer's investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 11 years since the subject device was manufactured.Based on the results of the legal manufacturer's investigation, a definitive root cause of the faulty power supply unit could not be identified.Based on the results of the legal manufacturer's investigation, a definitive root cause of the front panel led glowing could not be identified.However, it¿s likely the cause is related to a faulty printed circuit board.Olympus will continue to monitor field performance for this device.
