Philips has investigated this complaint.According to the additional information acquired, the system was in clinical use for a planned neurovascular procedure when the system displayed a "test shot underexposed" messaged during dsa (digitally subtracted angiography).The procedure was completed with a delay of 15 minutes.A philip remote service engineer (rse) analyzed the issue remotely and identified that the message was due to fluoroscopy being active for less than 3 seconds before dsa was started.No malfunctions of the system were identified via log file analysis.The system was returned to use in good working order.At the time this complaint was received, philips did not have enough information to exclude the potential for death or serious injury on recurrence, and as such the complaint was reported.Since that time, new information has been received which has led to the determination that this is scenario is not likely to cause or contribute to death or serious injury if it were to recur.The message displayed by the system was related to a workflow issue and no malfunction has been identified.Based on the investigation results, philips concluded that the complaint is not reportable.The codes were updated based on the investigation outcome.
