Event Description
|
It was reported that a patient underwent an atrial fibrillation (afib) ablation procedure with a decanav electrophysiology catheter and a foreign material issue was encountered.It was reported that there was an extra piece of plastic on the tip of the catheter.Caller stated they noticed it when the catheter was removed from the body.When the catheter was replaced, the issue resolved.There was no patient consequence.Additional information was received which indicating there was no damage that resulted in internal wires/braids being exposed.There were no lifted or sharp rings and no difficulty inserting or removing the sheath.The issue was further described as a ¿partial external artifact on the tip of the catheter.¿.

On 3-aug-2023, the bwi product analysis lab received the complaint device for evaluation.The product analysis has begun but is not completed at this time.When the investigational analysis has been completed, a supplemental 3500a report will be submitted.If additional information is received regarding this event, a supplemental 3500a report will be submitted to the fda.Manufacturer's ref.# (b)(4).

It was reported that a patient underwent an atrial fibrillation (afib) ablation procedure with a decanav electrophysiology catheter and a foreign material issue was encountered.It was reported that there was an extra piece of plastic on the tip of the catheter.Caller stated they noticed it when the catheter was removed from the body.When the catheter was replaced, the issue resolved.There was no patient consequence.Device evaluation details: the device was returned to biosense webster inc (bwi) for evaluation and the evaluation has been completed.A visual inspection evaluation of the returned device was performed following bwi procedures.Visual inspection revealed a plastic foreign material attached to the first electrode of the device.For this reason, a fourier-transform infrared spectroscopy (ft-ir) analysis was requested, and the results revealed that the foreign material is mainly a biological-based material according to the characteristic absorption bands.Foreign material can be associated mainly with biological material from the patient since it has already been used.A manufacturing record evaluation was performed for the finished device batch number, and no internal actions were identified.The issue reported by the customer was confirmed.It should be noted that product failure is multifactorial.As part of biosense webster¿s quality process, all devices are manufactured, inspected, and released to approved specifications.If additional information is received regarding this event, a supplemental 3500a report will be submitted to the fda.Manufacturer's ref.# (b)(4).

It was reported that a patient underwent an atrial fibrillation (afib) ablation procedure with a decanav electrophysiology catheter and a foreign material issue was encountered.It was reported that there was an extra piece of plastic on the tip of the catheter.Caller stated they noticed it when the catheter was removed from the body.When the catheter was replaced, the issue resolved.There was no patient consequence.Device investigation details: the device investigation has been completed which included performing a manufacturing record evaluation (mre).The manufacturing record evaluation performed for the finished device with lot number 30997518m identified no internal actions related to the reported complaint condition.Since no device has been received for analysis, no product investigation can be performed, and the customer complaint cannot be confirmed.However, if the product is received at a later date, the investigation will be updated as applicable.As part of our company quality system process, all devices are manufactured, inspected, and distributed to approved specifications.Additionally, the manufactured date and expiration date have been provided.Therefore, fields d4.Expiration date and h4.Device manufacture date have been populated.If additional information is received regarding this event, a supplemental 3500a report will be submitted to the fda.Manufacturer's ref.# (b)(6).
