The evaluation thus far has confirmed that the hvps board was defective.The evaluation is ongoing.A review of the manufacturing and quality control review was performed for the subject device and shows no non-conformities with respect to the described problem.The device history records indicates the device was manufactured according to valid instructions and met all specifications.The investigation is ongoing and follow up with the user facility is currently being performed.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Event Description
The customer reported to olympus, the generator had a bending problem.The issue was found during maintenance.Inspection and testing of the returned device found a permanent reboot message, error e433, resulting from a defective high voltage power supply (hvps).There were no reports of patient harm.This medical device report (mdr) is being submitted to capture the reportable malfunction found during evaluation.
 
Manufacturer Narrative
Correction to g3 of the initial medwatch.The aware date should be 09-mar-2023.This report is being supplemented to provide additional information based on the legal manufacturer's final investigation and device evaluation.The device was returned to olympus for inspection, and the customer's complaint was confirmed.Based on the results of the investigation, the root cause could not be determined.The event occurred due to a defective high voltage power supply (hvps) board, however, the exact cause of the defect could not be specified.It likely the hvps board failure occurred due to one of the following: the supply voltage from the hvps board was missing or too low (permanent error), and/or a short circuit of the metal¿oxide¿semiconductor (mos) transistors (permanent error).In addition, the following non-reportable malfunctions were found during the device evaluation: dust inside the device, scratches on the housing cover, scratches on the housing chassis, and cracks in the front panel.Olympus will continue to monitor field performance for this device.
