Event Description
The customer reported the evis exera iii xenon light source connector fitting has a deformed plug iron and a broken piece of plastic.The event occurred during preparation for use and there was no patient harm or user injury reported due to the event.
 
Manufacturer Narrative
The device was returned for investigation.Upon evaluation of the device, cracked connector, front panel crack, top cover paint peeling, chassis deterioration, and rear panel depressed were observed.The reported malfunction was due to wear/tear and mishandling.The investigation is ongoing; therefore, the root cause of the reported event cannot be determined at this time, however, if additional information becomes available, this report will be supplemented accordingly.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation it was confirmed that the imaging issue was caused by damaged scope socket.However, the cause of the damaged scope socket was not established at this time.The occurrence of the reported problem can be prevented by adhering to the instructions for use (ifu) which states the following: ¿do not apply excessive force to the light source and/or other instruments connected.Otherwise, damage and/or malfunction can occur.¿ olympus will continue to monitor field performance for this device.
