The device was returned to olympus for evaluation and the customer's allegation was confirmed.The device evaluation found the front panel was damaged.The chassis was deformed.The power switch was an old version.The scope socket was bad.The worn out scope socket slider switch causes intermittent use of high intensity mode.The turret first wheel out of alignment.Air pump has unusual sound and water invasion inside.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation.
 
Event Description
The customer reported to olympus, the power switch on the evis exera iii xenon light source was not turning on the device.It was reported that the switch was broken or worn out.The issue was found during preparation for use.There were no reports of patient harm.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.Please see updates to h6 and h10.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 9 years since the subject device was manufactured.Based on the results of the investigation, it¿s likely the device did not power on due to a failure of the power switch harness.The final root cause of this event was unable to be identified.Olympus will continue to monitor field performance for this device.
