|
The customer reported to olympus, that the evis exera iii xenon light source's fan motor was worn out.It was added, that the device was making noises and that the electrical board lights were all blinking.There was no report of patient harm.

The device was returned to olympus for evaluation of the reported issue.The allegation regarding the fan making noise was not confirmed.However, the blinking lights were confirmed, due to a faulty socket slider switch.Other findings include a cracked front panel mouth, a rusted chassis, and a noisy air pump.Additionally, the device's non-olympus lamp life meter was reading at 499 hours and 32 minutes.The light output measured out of range.And a replacement to the olympus xenon lamp was recommended for optimal performance.The investigation is ongoing.And a supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation, and to correct information provided on the initial report.Based on the results of the investigation, it was determined that phenomenon, ¿all lights/lamps blink" was reproduced, and it was due to a slider switch failure in the socket.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 8 years since the subject device was manufactured.Olympus will continue to monitor field performance for this device.
