It was reported that a peritoneal dialysis (pd) transfer set was difficult to disconnect from the patient line of a homchoice cassette which resulted in a separation between the female connector (navy blue portion) and the main body (light blue portion) of the transfer set.This issue occurred while attempting to disconnect from the cassette during pd therapy.To resolve this issue, the transfer set was replaced.There was no report of patient injury or medical intervention associated with this event.No additional information is available.
