Medtronic received information that during the implant of this 24mm mitral mechanical valve, it was explanted and replaced with a 24mm valve of the same exact make and model.The reason for the replacement was reported as the handle of the valve holder was sewn up and the thread broke.No additional adverse patient effects were reported.

Event Description

Medtronic received information that during the implant of this 24mm mitral mechanical valve, it was not able to be implanted and replaced with a 24mm valve of the same exact make and model.The reason for the replacement was reported as the handle of the valve holder was sewn up and the thread broke.It was also reported that when the valve suture was placed on the mechanical valve, the suture thread that fixed the valve on the valve holder was broken and the valve could not be pushed to the designated position accurately.No additional adverse patient effects were reported.
