The device was returned to olympus for evaluation.The customer¿s allegation was not confirmed.The inspection found that the device had a bad connector, which caused the image to drop out with an error code b30 being observed.The cause of the reported issue is most likely due to normal wear and tear of the device.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation.
 
Event Description
It was reported by the customer that the evis exera iii xenon light source had a bad socket, corrosion on the pin and exhibited scope communication error b30.There were no reports of patient harm or impact associated with the reported event.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.Please see updates to h4, h6 and h10.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 9 years since the subject device was manufactured.Based on the results of the investigation, it¿s likely the b30 error was due to a faulty scope connector.The final root cause of this event was unable to be identified.Olympus will continue to monitor field performance for this device.
