The subject device was received at an olympus service center for evaluation.During inspection and testing, service found the lamp heat sink was burnt, and lamp post was burnt.The front panel mouth is cracked.Worn out socket slider switch causes intermittent use of high intensity mode.The investigation is ongoing; therefore, a root cause of the reported event cannot be determined at this time.If additional information becomes available, this report will be supplemented accordingly.
 
Event Description
The customer reported the evis exera iii xenon light source showed sparks inside the device and the heat sink had burn damage.The customer reported these issues was identified prior to procedure.No adverse effects to patient or users reported.
 
Event Description
Additional information: the patient was scheduled for diagnostic gi/endoscopic examination.No other devices were involved.The device (light source) was swapped out resulting in "little to no delay." the procedure was completed as scheduled.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 9 years since the subject device was manufactured.Based on the results of the investigation, a definitive root cause could not be established.Olympus will continue to monitor field performance for this device.
