An olympus service representative spoke with the customer over the phone, and performed troubleshooting of the device.The customer was given instructions for resetting their device, and they were instructed to call back in with any further concerns.No further communications have been received from the customer.The device was not returned for evaluation.If the device is returned it will be evaluated and a supplemental report will be filed.
 
Event Description
The company representative reported on behalf of the customer that the evis exera iii xenon light source caused the monitor to display error b30 (scope socket error).No adverse effects to patient reported.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 10 years since the subject device was manufactured.Based on the results of the investigation, it is likely that the following parts may be faulty and led to the malfunction: ·thermo switch ·main kiban ·lamp house ·xenon lamp ·ignitor ·dengen u however, a definitive root cause could not be established, as the device was not returned for evaluation.Olympus will continue to monitor field performance for this device.
