Event Description
It was reported to olympus that the evis exera iii xenon light source had an error code e212.The event occurred during preparation for use.There was no patient harm associated with the event.The device was evaluated by a field service agent, and the xenon lamp is burnt.The mdr (medical device report) is being submitted to capture the reportable malfunction found during the device evaluation.
 
Manufacturer Narrative
The device was evaluated by a field service agent, and the customer¿s allegation was not confirmed that an e212 error had occurred.It was found the xenon lamp was burnt and they replaced the xenon lamp with a new one.The investigation is ongoing, and a supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.Based on the results of the investigation, the device was not returned, and a root cause could not be identified.The burnt xenon lamp was replaced.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 11 years since the subject device was manufactured.The cause of the reported event cannot be conclusively determined.Olympus will continue to monitor field performance for this device.
