|
This is filed to report torn material.It was reported this was a mitraclip procedure to treat mixed mitral regurgitation (mr) with a grade of 3.An xtw clip was inserted into the left atrium (la).The clip opened without issues.However, while closing the clip, it was observed one clip arm was not at the same angle to the shaft as the other clip arm.Therefore, the clip closed and replaced.Two clips were then implanted, reducing mr to a grade of 1-2.There were no adverse patient effects and no clinically significant delay in the procedure.The returned device analysis reported the clip introducer was torn.No additional information was provided.

All available information was investigated, and the reported uneven clip arms was confirmed via returned device analysis.Additionally, a bent threaded stud and torn clip introducer were observed.A review of the lot history record revealed no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no similar complaints reported from this lot.Based on available information and the returned device analysis, the cause of the observed bent threaded stud was unable to be determined.The reported uneven clip arms was a cascading event of the observed bent threaded stud.The observed torn clip introducer was due to post-procedural handling of the device and are unrelated to the reported event.There is no indication of a product quality issue with respect to manufacture, design, or labeling.
