An olympus field service engineer (fse) placed a parts order and will schedule the repair once all items are available.The investigation is ongoing, and a definitive root cause of the reported event cannot be determined at this time.If additional information becomes available, this report will be supplemented accordingly.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation and device evaluation.The device was not returned, however, the customer's complaint was confirmed based off the fse (field service engineer) evaluation on site.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, it is likely that the event occurred due to the user handling: the user closed the disinfection bottle drawer while a pair of bottles was not set properly.As a result, the bottles contacted with the drawer.The event can be prevented by following the instructions for use which state: "chapter8 troubleshooting and repair if any irregularity is detected during an inspection or if the device is clearly malfunctioning, do not use it.Contact olympus for repair." olympus will continue to monitor field performance for this device.
