Event Description
It was reported that on an unknown date, while putting in a lumbar screw we had both a t20 navigated and non navigates driver break.The driver tips sheared off.The tip is the standard expedium t20 was left in the screw with in the patient, and a rod was placed over it.Patient consequence and surgical delay was unknown.This complaint involves two (2) devices.This report is for one (1) 5.5 nav driver - shaft t20.This is report 2 of 2 for complaint (b)(4).
