Event Description
A customer reported the cv-180 evis exera ii video system center ceased to function while in use.The unit overheated and shut down during a procedure.The procedure was completed with another device.There were no reports of patient harm.
 
Manufacturer Narrative
The device was returned to an olympus repair facility, and an evaluation of the device was performed.Upon inspection and testing of the unit, the reported problem that the device ceased to function while in use was not confirmed.Additional inspection revealed that the video connector was in fair condition, the nbi (narrow band imaging) was up to date, and the system software was up to date.The investigation is ongoing.If additional information becomes available, this report will be supplemented accordingly.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, a probable cause could not be identified.Olympus will continue to monitor field performance for this device.
