During device evaluation at olympus, it was found the complaint was not confirmed and no fluid invasion was found.Evaluation did find the front panel mouth was cracked, the top cover was rusted, the bottom and front side chassis was rusted, there was unknown residue on the rear panel, the socket slider switch was worn out which caused intermittent use of high intensity mode, the scope socket was corroded, and a non-olympus lamp life meter was reading over 500 hours.This event is under investigation.A supplemental report will be submitted upon receiving additional information.
 
Event Description
The customer reported to olympus that the evis exera iii xenon light source had fluid invasion.The issue was found during the preparation for use.There were no reports of patient or user harm associated with this event.  .
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, it is likely that the customer misidentified the phenomenon since inside of the device was checked but there was no trace of fluid invasion.Olympus will continue to monitor field performance for this device.
