Event Description
A distributor reported on behalf of the customer that, during a pre-use inspection of the device prior to use in an unknown therapeutic procedure, the image produced by the customer¿s endoeye flex deflectable videoscope would not appear correctly; when angulated, the color of the displayed image would change to purple, yellow, and other unexpected colors.The procedure was completed when the customer changed to a 10 mm scope, and the issue did not cause any delay.There were no reports of patient or user harm associated with this event.
 
Manufacturer Narrative
The device was returned to an olympus repair facility, and an evaluation of the device was performed.During the evaluation, the customer's originally reported issue of irregular color tone was not confirmed.The following additional findings were also noted: assorted scratches and chips, wear of the angle wire causing bending angle in the up direction to not meet the standard value, and loss of water tightness due to damage on the light guide cover glass.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 11 years since the subject device was manufactured.Based on the results of the investigation, the customer's issue could not be confirmed.The reported failure could not be reproduced.Olympus will continue to monitor the field performance of this device.
