Event Description
|
It was reported when performing a therapeutic transurethral plasma resection of the prostate the device was used to cut the tissue, it was found that the tissue could not be cut off when it reached to the root of the tissue, and the wire at the distal end was burnt black.The device was replaced and the intended procedure was completed with a similar device.There were no reports of patient harm or delay.No user injury reported.

This report is being supplemented to provide additional information based on the legal manufacturer's investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, the root cause of the burnt wire was wear and tear.The loop at the distal end of the electrode wears out during use and may break, burn or melt.Olympus will continue to monitor field performance for this device.
