The company representative able to replicate the reported event.The core module was replaced to address the issue.The system was tested and found to meet product specifications.A non-conformance based review of the serial number was performed and did not reveal any potential contributing factors to the reported complaint.A manufacturing device history record (dhr) review was performed prior to product release to ensure that the product was manufactured in compliance with the device master record.Based on the assessment, the product met release criteria.A review for complaints reported against this serial number was performed.No similar complaints were reported for the product serial under investigation.The root cause of the reported event is attributed to the nonconforming core module.Manufacturer will continue to monitor data for evidence of adverse trending and take further action, as appropriate.The manufacturer internal reference number is: (b)(4).
 
Event Description
A customer reported that an ophthalmic operating console exhibited a laser error.Procedure details and patient impact have not been provided.Additional information has been requested but none received till date.
 
Manufacturer Narrative
The company representative was able to replicate the reported event.The laser module core assembly was replaced to address the issue and was returned for testing.The system was tested and found to meet product specifications.A non-conformance based review of the serial number was performed and did not reveal any potential contributing factors to the reported complaint.A manufacturing device history record (dhr) review was performed prior to product release to ensure that the product was manufactured in compliance with the device master record.Based on the assessment, the product met release criteria.A review for complaints reported against this serial number was performed.No similar complaints were reported for the product serial under investigation.The module core assembly was visually inspected for non-conformities and none were found.After installing the sample into a calibrated system.The system booted up, the system generated system message (sm) the laser then shut down.After recalibrating the photo-monitors (pmons) in the core module the test was re-performed and no system message appeared.The unit additionally passed energy verification test with no system errors.The root cause was determined to be the pmons, which were out of calibration.The root cause of the reported event is attributed to the component, which were out of calibration within the laser module core assembly.Manufacturer will continue to monitor data for evidence of adverse trending and take further action, as appropriate.The manufacturer internal reference number is: (b)(4).
