Event Description
The customer reported the evis exera iii xenon light source did not work with some gastroscopes.The customer reported this issue occurred before the procedure and then again in the middle of a diagnostic and therapeutic colonoscopy/gastroscopy.The customer reported the procedure was delayed for 15 minutes while patient was under sedation with propofol.The procedure was completed with similar device with no adverse effects to patient.The device was returned for evaluation.The evaluation showed that the socket connector had sustained fluid ingression causing connector issues.This medical device report (mdr) is being submitted to capture the reportable malfunction found during evaluation.No adverse effects to patient reported.
 
Manufacturer Narrative
The device was returned for evaluation.In addition to the fluid ingression in the connector, fluid ingression was found in the front panel.The examination showed the connector portion of the circuit board was in poor condition and the device chassis was rusted.The investigation is ongoing.A supplemental report will be submitted upon completion of investigation.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, a probable cause could not be identified.Olympus will continue to monitor field performance for this device.
