Event Description
A non health care professional reported that after implantation of intraocular lens (iol), the lens was damaged.The lens was explanted in a secondary procedure and replaced with a monofocal lens.The clinical reason for explant was lens damage.Additional information was received stating that the injector plunger has damaged the lens.Additionally, the patient also experienced vision glare and secondary image.Hence the lens was explanted.The patient symptoms was resolved and the patient was fully recovered.There was no patient harm.

A sample was not received at the manufacturing site for evaluation for the report of lens damaged by the injector plunger and explant iol; therefore, the condition of the product could not be verified.No lot number was identified with this complaint; therefore, a device history record review could not be conducted.A sample was not received at the manufacturing site and no lot information was provided; therefore, the root cause for the customer complaint issue cannot be determined.The manufacturer internal reference number is: (b)(4).
