Event Description
An attempt was made to use one resolute onyx rx coronary drug-eluting stent to treat a lesion.It was reported that the stent came off the balloon catheter in the guide catheter.The guide catheter was removed with the stent inside.It was also detailed that there were two wires twisted in the guide catheter.The procedure was completed with another medtronic stent.No injury to the patient was reported.
